Consent

I give consent for staff at Camp Gan Izzy Essex to:

1. Administer First Aid & Emergency Care - Administer first aid to my child and, if necessary, seek emergency medical treatment and/or take my child to hospital in the event of an accident or emergency. (Please indicate in the medical section above if your child has an allergy to plasters.)
2. Trips & Outings - I give permission for my child(ren) to take part in camp outings, trips and visits to local parks, understanding that normal risks associated with such activities may apply. I also authorise Camp Gan Izzy Essex to sign any required waivers on my behalf where necessary.
3. Damage to Property - I understand that I may be held responsible for any damage to camp property caused by my child.
4. Drop-Off & Collection - I understand that children may only be dropped off and collected by nominated adults. Children attending Teen Camp may travel to and from camp independently and sign themselves in/out unless parents/guardians inform us otherwise.
